Health Education/Additional
Programs:
- Reach Out and
Read - A program that seeks to make early literacy a standard part of
pediatric primary care. Pediatricians encourage parents to read to their
children and give out age appropriate books to take home at all pediatric well
visits from six months to five years of age. A HealthFirst
volunteer visits the Center once a week to read to children as they wait for
their appointments.
- QuitWorks - A
telephone counseling, information, and referral service for Massachusetts
residents who want to stop smoking. QuitWorks is part of the
Try To
Stop Tobacco Resource Center of Massachusetts.
- Project Bread -
This program provides HealthFirst with funds to purchase
supermarket food vouchers for families who are experiencing food insecurity, as
determined by guidelines of the project. These vouchers are one time only and
patients are referred to the Social Worker for future resources.
- Women's Health
Network - Services provided by
Saint Anne's
Hospital to assist women without insurance in receiving annual mammograms
and pap smears. Services are free to women who meet program
guidelines.
- Influenza
Clinics - Annual flu vaccination clinics held at the health center for the
general public.
- Immunization
Clinic - Co-sponsored by HealthFirst's WIC and Pediatric
Departments for children up to age five.
- Chest Clinic -
HealthFirst works in conjunction with the Fall River Health
Department to provide a chest clinic for patients being treated for
tuberculosis. Twice a month, patients being treated by the Health Department
meet with the city's pulmonologist (physician specializing in the treatment of
diseases of the lungs) and nurses at HealthFirst. Patients
receive chest X-rays, laboratory services and medications in a convenient,
clinic setting.
- HealthFirst Family Care Center is a member of the Massachusetts
Health Disparities Collaboration which focuses on chronic illnesses.
HealthFirst is currently addressing diabetes. The goal of
both the Collaborative and HealthFirst is to improve patient care and clinical
outcomes for patients living with diabetes. HealthFirst
patients who are diabetic receive comprehensive care that includes, in addition
to direct patient care provided by the physician, relevant health and diet
education, medication review and instruction, and social services from a team
of nurses, nutritionists and social workers trained in the management of
chronic disease.
