Christoph Sahar, MD, FAAFP - Medical Director
Christoph Sahar, MD, FAAFP loves teaching and being a mentoring resource to other providers. His medical interests include: providing comprehensive care for families, addiction medicine, adolescent health, behavioral medicine, and population health. He studied Computer Science at Brown University, and attended UMDNJ-Robert Wood Johnson Medical School (now Rutgers) in New Jersey. Dr. Sahar’s administrative duties limit the size of his practice. Between caring for his established adult patients who followed him to HealthFirst, and assisting other providers deal with their most complex adults, he is only taking new patients up to age 21 years of age, with the goal of adding their parents (if desired), as his duties permit.
Pam Bermudez, RD, CDE
Pam Bermudez, RD, CDE has been a registered dietitian for over 30 years. Her first experience was in pediatrics and women in pregnancy, working at Boston City Hospital and then Women and Infants Hospital in Providence. Fifteen years ago, Pam became certified as a diabetes educator, and then left Women and Infants and became part of a team of nurses and dietitians for the Southcoast Hospital Group Diabetes Education program. In 2011 she joined HealthFirst to be part of the Care Management Department.
Kevin Harkins, PA
Kevin Harkins, PA is very happy to be back working at HealthFirst after 8 years away in western Massachusetts. Most recently he has been working at Tobey Hospital emergency department. He graduated from the Northeastern University PA program in 1993. Kevin welcomes the opportunity to care for community members of all ages.
Karla Fuentes, MD
Karla Fuentes, MD enjoys working with children from newborn to adolescents. Dr. Fuentes has a special interest in caring for newborns and promoting breastfeeding.
James “Jim” Hodson, FNP
James “Jim” Hodson, FNP graduated from Southern Adventist University in Tennessee with his bachelor’s degree in nursing before earning his Masters of Science from the University of Massachusetts at Lowell. Jim is a board certified Family Nurse Practitioner. His passion is disease prevention, treatment, and the health education of his patients of all ages. In his spare time Jim enjoys landscaping, world traveling, raising canaries, and learning new languages.
Mary Kavanaugh, RN, CDE, MTTS
Mary Kavanaugh, RN, CDE, MTTS is a 59 year old mother of four grown children. She received her Bachelor of Arts degree from Providence College in 1978 and held various jobs and volunteer positions while her children were growing up. Mary began Nursing School at UMass Dartmouth in 2005 and graduated with a Bachelor of Science degree in 2008. She worked as a School Nurse in the New Bedford public school system for three years and began at HealthFirst in 2011. Mary is a Care Manager who works with patients to help them manage chronic illnesses, mainly diabetes, asthma, and hypertension. She recently received her Diabetes Educator Certification, and Massachusetts Tobacco Treatment Specialist Certification from the University of Massachusetts Medical School. Mary counsels individuals and groups in techniques and skills to help them stop smoking. She was trained as a Patient Navigator by the Care Coordination Program sponsored by the Massachusetts Department of Public Health.
Kristen Meiggs, FNP-C
Kristen Meiggs, FNP-C is a board certified family nurse practitioner working with HealthFirst Family Care Center as a primary care provider for people of all ages. She received her first degree in Nursing from Cape Cod Community College in Barnstable, MA where she went on to work in a community health center, as well as, for local towns as a town nurse providing immunizations and promoting health education. She received her Bachelors of Science degree in Biology and Psychology from the University of Massachusetts Dartmouth. She worked, both privately and for school systems, as a behavior modification specialist for many years helping children with autism spectrum disorders. Finally, K. Meiggs went on to Simmons College in Boston, MA where she earned her Masters in Nursing with a focus on family medicine. Since that time she has worked in Otolaryngology and Family Practice. Kristen is dedicated to providing her patients with the utmost care through a mutual goal building process between patient and provider.
Jessica Millett, RN
Jessica Millett, RN started her career in the health care field here at HealthFirst as a new medical assistant. She worked for HealthFirst from 2006-2012 as an MA. During this time she was also attending Bristol Community College part-time to become an RN. She now has two years of experience working as a dialysis RN. She recently re-joined HealthFirst as an RN, in the Complex Care Management Department, where she works with patients who have chronic illness, both in and out of the clinic. Her primary role is to educate patients on chronic disease management. Jessica often performs this education in the patient’s home, due to their transportation issues. Asthma, diabetes and heart disease are the most common chronic diseases that are managed in this department.
Kathryn Reynolds, ANP
Kathryn Reynolds, ANP is a certified Adult Nurse Practitioner seeing patients ages 18 and up and has been practicing since 2008. She graduated from Colgate University with a degree in English and then returned to school for her Master’s degree in Nursing at MGH Institute of Health Professions. Clinical interests include wound care, which she is certified in, as well as nutrition for disease prevention and management.
Michelle Boardman, MSW, LICSW
Michelle Boardman, MSW, LICSW is a Licensed Independent Clinical Social Worker available to all patients for counseling services through the care management department. She earned a master’s degree in social work from the Boston University School of Social Work in 2003. Her bachelor’s degree was earned in Sociology at UMass Dartmouth in 1998. Michelle’s experience includes working with individuals struggling with mental illnesses, medical illnesses, and life cycle changes. Michelle’s passion is geriatrics and working with adults. Her goal as a clinical social worker is to provide quality behavioral health services to patients of HealthFirst.
Katelynn Rei, PNP
Katelynn Rei, PNP is a Pediatric Nurse Practitioner (PNP) who cares for all children – newborns through adolescents. She graduated from Yale University in May 2013 and has been employed at HealthFirst Family Care Center since July 2013. Katelynn enjoys building relationships with her patients and their families in the Primary Care setting.
Gloria Mercado, MD
Gloria Mercado, MD is a graduate of the University of the East Memorial Medical Center in the Philippines, and has been practicing General Medicine in the United States for over 30 years. She has been at HealthFirst for 13 years dealing mostly with patients with acute and chronic illnesses. Dr. Mercado listens, cares, and works with her patients to resolve all of their medical issues. We are extremely happy she is part of the HealthFirst medical team.
Daniel Simoneau, ANP
Daniel Simoneau, ANP is a nurse practitioner that specializes in adult health. Daniel graduated from the University of Massachusetts of Dartmouth where he also spent time as a clinical instructor of undergraduate nursing. Daniel has a passion for helping others and looks forward to caring for the local residents of the greater Fall River area for years to come.
Katie Tavares, FNP
Katie Tavares, FNP Katie graduated from Assumption College with a bachelor’s degree in Biology & Psychology prior to earning her Master’s Degree of Science in Nursing and earning the distinction of Certified Family Nurse Practitioner. As a Family Nurse Practitioner, Katie has advanced education necessary to manage acute and chronic illnesses across the lifespan. Katie’s goal is to help others in the community live healthier lives through health education, disease prevention and counseling. In her spare time, Katie enjoys being with her friends and family, reading, traveling to new places, walking her dogs and gardening. Katie is dedicated to providing the highest quality care to her patients and is passionate in her belief that health is a human right.
